IMPORTANT NEXT STEPS:

 

• Print and read through the Terms of Participation form (if you have not done so previously)

 

 

 

• Fill out page 2 and sign document

 

• Scan or take a photo of the signed form and email it to CulinaryExp@gmail.com (2 weeks prior to class)*

 

• Bring signed, two page document to your cooking class*
